Если представить всю работу хирурга-эстетиста как айсберг, самую большую, подводную его часть (90%) составляет консультационная работа с пациентом — составление плана действий, прогнозирование результата, коррекция ложных ожиданий, — тогда как верхушкой (10%) становятся непосредственно механические действия доктора, предусмотренные той или иной процедурой. Секретами успешного взаимодействия с пациентом, гарантирующими положительный исход любой процедуры, делится пластический хирург, руководитель “Эстетической медицнской клиники доктора Крамера”, член Израильского общества пластической и эстетической хирургии Эяль Крамер.

Расскажите немного о себе: что Вы любите, знаете и умеете лучше всего? Чем увлекаетесь?

Если у Вас нет страсти к тому, что Вы делаете, бросайте это немедленно. Особенно, когда речь идет о пластической хирургии, в которой так много подводных камней: сложные пациенты, необходимость учить новое в режиме нон-стоп и так далее. Что касается лично меня, моя специализация, страсть и увлечение — это процедуры, связанные с преображением всего того, что находится выше ключиц. Я выполняю процедуры на лице, шее и руках, но конечно же моя главная стратьсть и мой главный интерес — это область лица.

Как Вы справляетесь с неоправданными ожиданиями пациента? 

Неоправданные ожидания не зависят ни от возраста, ни от пола — в первую очередь, это — результат работы доктора со своим пациентом и объяснение того, что будет хорошо выглядеть конкретно на его лице.

Предоперационные консультации занимают у меня больше времени, чем сама процедура, потому что разговор — ключевой момент, залог успеха любого взаимодействия доктор-пациент.

Во время консультаций важно объяснить, чего стоит ожидать, что будет хорошо выглядеть на лице каждого индивидуально взятого пациента. Многие считают, что всякая морщина на лице — это непозволительный ужас, но лично я уверен, что некоторые возрастные складки могут быть красивыми. Доктору важно сконцентрироваться на естественном результате в долговременной перспективе. Избыточное использование филлеров, или чрезмерный лифтинг в нашем деле будут выглядеть нелепо и неуместно.

Если я вижу, что у моего пациента есть настойчивые нереалистичные ожидания по отношению к процедуре, я отказываюсь от работы с ним. На сегодня пластическую хирургию вряд ли можно назвать дефицитной: в мире сотни докторов, которые согласятся сделать с лицом то, что им скажут, даже если пожелания выходят далеко за грань здравого смысла. Но я глубоко убежден, что наша задача, как специалистов — помочь людям принимать любой возраст с достоинством. Всякие перегибы выглядят странно и никогда — привлекательно. Три основных принципа нашего успеха — это, во-первых, идти к цели шаг за шагом, во-вторых, использовать небольшие дозы продуктов, и, в-третьих, проводить коррекцию достаточно часто — уж точно не один раз в год. Поймите и примите тот факт, что главная наша задача — выглядеть естественно, а не смехотворно и неуместно, зато молодо.

С какими заблуждениями относительно процедур Вы сталкивались?

Самое распространенное заблуждение — мнение о том, что можно добиться полного устранения морщин в любом возрасте с помощью одного лишь филлера. Это неправда. Так нельзя обращаться с кожей.

Сейчас задачей хорошего пластического хирурга стало объяснение каждому пациенту того, что значит стареть естественно и достойно.

Если Вы будете вводить чересчур большие дозы филлеров, есть риск добиться обратного эффекта: выглядеть старше, чем Вы есть.

Что касается эстетичекой медицины, запомните одно простое правило: меньше — это больше. Ходите к доктору регулярно, постоянно ухаживайте за своей кожей, и она ответит Вам тем же — сиянием, здоровьем и отзвуком десятков ежедневных комплиментов.


Eyal Kramer: dealing with a patient, voice your own opinion, not the thing they want to hear from you

If you submit the whole work of any kind of aesthetic surgeon in a form of glacier berg, the biggest subsurface part of the iceberg (90%), will consist of patient management – mapping out a course of action, procedure prediction, compensating adjustment of false expectations, – whereas in contrast a silver of the full scope (10%) still remains with mechanical effort of the doctor, defined by the procedure itself.
Eyal Kramer, an experienced plastic surgeon, lead of “Aesthetic Medical Dr. Kramer's Clinic”, Israel Society of Plastic and Aesthetic Surgery member, kindly agreed to share his secrets of successful patient managing, guaranteeing a further positive outcome of any procedure.

Tell us about yourself: what are you fond of, what do you know best of all and excel in?

If you do not feel passionate about whatever on Earth you are doing, drop it off right this minute, immediately! Especially when it comes to plastic surgery – the field, where you can find thousands of pitfalls literally at every step: difficult patients, irrefutable need to upgrade yourself on a loop as a professional and so on and so forth. For my own part, my specialization, passion and finest hobby are procedures, related to the transformation of everything in human body, that is located above the collar bones. I perform face surgery, deal with neck and hand areas, but of course my main passion and my primary interest is the face.

How do you deal with your patient's false expectations?

False expectations are difficult to prevent, because they are totally unpredictable: they depend neither on age, nor on the patient's gender – first of all, this is the result of a doctor and patient intercommunion and an informative explanation of what to expect to appear on the face after the scheduled treatment. Moreover, this is all about timing and teaching the patient of what is appropriate for them.

Pre-operative assessment and planning takes me more time than any procedure itself. The reason why? Because the conversation is a key insight, it is a passport to success of any prosperous doctor and patient interaction.

It is vitally important to explain what to expect, what is going to look properly for this particular face structure right in the course of the consultation. People tend to think that each and every wrinkle that happens to show up on the face is an unacceptable nightmare, but personally I believe that some facial lines are even beautiful. The plastic surgeon has to concentrate on the natural procedure outcome and to see it through in the long range. Both fillers over-treatment and excessive lifting will look ridiculous and totally inappropriate.

Given that I see that my patient has already got any kind of persistent unrealistic expectations concerning our mutual goal – the scheduled procedure, – I categorically refuse to work with them. The good news is, today the field of plastic surgery can be hardly called difficult-to-obtain: there are hundreds and hundreds of doctors, who will agree to transform the face in any way, which is acceptable for their patient – even if the voiced request is far beyond the right side of common sense. I am strongly convinced that any plastic surgeon's professional target is to help people age gracefully. All the excesses do look strange and never seem to attract anybody's attention in a good way. Three pillars all the plastic surgeons lean on performing their work successfully are: firstly, to go straight to the chosen goal step-by-step, secondly – to use small dosage of the product to give their patient the most natural look, and thirdly – to plan and carry out the correction at least once a year. Please, get your head around the fact that our main task is to look unforced and natural, rather than to appear a ridiculous and inappropriate, but quite young human being.

What kind of misconceptions concerning procedures did you have to deal with?

The most common misconception is the one that you can get rid of all the wrinkles on your face at any age using filler only. This is not true. You cannot do that to your skin.

Nowadays the main task of any plastic surgeon is to explain each of their patients what does it really mean to age better, gracefully and naturally.

If you inject too much filler, there occurs a risk of having totally reverse effect: you might look even older than you currently are.

With regard to medical aesthetics, you should keep in mind only one simple rule: less is more. Visit your doctor regularly, provide your skin with proper constant care – and it will respond to you with a mirror image. Behaving in this way, you will make your skin glowing, healthy and bringing you alluring echo of dozens of compliments on a daily basis.
